Outreach Facilitator, DisabilityLocation All AucklandAuckland Job type Permanent Salary to Category Medical > Other » Work making a difference in Peoples lives every day » Great career progression opportunities » Flexibility of work environment Outreach Facilitator The Outreach Facilitator is responsible for managing the information and planning process for clients who are in public hospitals. The role involves ensuring that clients are engaged with the appropriate services and support at the right time, while maintaining oversight and ensuring consistent achievement of desired outcomes for clients and their families/whanau. The Outreach Facilitator will provide professional client services according to specified guidelines and internal processes, conducting assessments, discussing privacy and compliance requirements, collaborating with relevant parties, and coordinating support packages. The role also involves maintaining documentation, authorizing access to funded and non-funded support, managing service gaps and failures, and reviewing and updating support plans. Responsibilities: » Conduct professional support needs assessments/reassessments based on contract service specifications, guidelines, and internal processes. » Identify and prioritize client support needs, educating clients on the user-driven process. » Perform self-assessment validations, guided assessments, and in-person assessments as required in the Pathways process. » Engage all relevant parties in the needs assessment process as necessary. » Collaborate with clients to create and explore support options, consulting and agreeing on support packages within the decision framework. » Support service calls and adjust support plans as needed. » Maintain high-quality documentation and records in accordance with documented processes and best practices. » Authorize and coordinate access to funded and non-funded disability or personal health support. » Identify interim and crisis service provision for clients and families, advising or escalating to the appropriate external party as needed. » Update Support Plans based on holistic reviews of the client's situation. » Ensure referrers and specialists receive information about the client's support package with the client's permission. Qualifications and Experience: » Bachelor's degree in a relevant field (e.g., social work, psychology, healthcare management) or equivalent experience. » Experience working in a similar role, preferably in a healthcare or social services setting. » Knowledge of relevant legislation, privacy regulations, and compliance requirements. » Strong assessment and case management skills. » Excellent communication and interpersonal skills to engage and collaborate with clients, families, and external stakeholders. » Ability to prioritize tasks and manage time effectively in a fast-paced environment. » Attention to detail and ability to maintain accurate documentation. » Experience in crisis intervention and risk management. » Familiarity with disability or personal health support systems is preferred.
