Ara Poutama Aotearoa (Corrections) Health Services lead the delivery of patient-focused primary health care services to tangata within corrections facilities across Aotearoa. H?kai Rangi, our over-arching strategic direction, places the focus on health at the heart of our business. Building our frameworks from a Matauranga M?ori basis is our focus to drive and support equity for M?ori, which in turn will contribute to achieving our results of reducing reoffending.

The High Dependency Unit (HDU) located at Rimutaka Prison is a nationally shared resource for older people in the management of Ara Poutama Aotearoa. The HDU accommodates up to 30 persons who have age-related or complex health and discapability needs. The Health Centre Manager has oversight of the HDU while the day-to-day running is managed by the adjunct Health Centre Manager.
We are seeking for a highly skilled Social Worker to work with the men in our management who have health needs requiring specialised expertise. In this function you will streamline an integrated approach to admission and exit of people in our management, to and from the HDU. Your responsibilities in this function will include:
   » Working closely with Case Managers to ensure the reintegrative needs of each person are identified and addressed as required.
   » contact with and support the inclusion of whanau in the identification of clinical history, planning of care, visiting, and release planning.
   » Working with the people in care to facilitate Welfare Guardianship or Enduring Power of Attorney where appropriate
   » contact with residential care facilities where required to support the prosperous transition of people in our care into the community.
   » Work with men in the HDU by identifying their psychosocial, mental and emotional needs along with providing, developing, and/or aiding the access of services to meet those needs.
   » Be responsible for fostering an environment and routines that support men in care to maximise their individuality, independence and dignity. This environment will provide men with the highest practical level of physical, mental and psychosocial well-being and quality of life

We invite applications from suitably qualified candidates who can demonstrate strong alignment to our kaupapa and our organisation values. The panel will look for evidence within your cover letter as to how you meet the following lead Requirements:
   » A Social Work qualification recognised by the Aotearoa NZ Association for Social Workers Registration Board;
   » Current registration and practicing certificate
   » Strong clinical practice in social work, counselling, or other psychological therapies and approaches;
   » practice in working within the aged care sector and knowledge of NASC and InteRAI is preferable but not essential
   » practice working with Kaupapa M?ori services, and commitment to continuing personal development in this area;
   » Can operate effectively in a M?ori cultural setting and has an knowledge of the Treaty of Waitangi and how Information technology relates to health and wellbeing;
   » capability to make sound professional judgements tailoring solutions to individual circumstances; and
   » Strong people engagement, relationship-building and collaboration expertise.
The hours of work for this position are Monday to Friday core business hours. We are happy to consider applications from candidates who are looking to work either full time or part time with a minimum of 30 hours per week. The income range is: $58,481-$78,347 per annum.
