District Nurse Positions - Community Health Wellington

Newtown, Wellington, New Zealand

Full-Time or Part-Time (1.0 FTE, 0.8 or 0.9 FTE)

Key Responsibilities:

Deliver comprehensive district nursing services to patients in the community setting.

Provide specialized care, including intra venous therapy, palliative care, wound care, medication management, stoma care, and respiratory care.

Support patients transitioning from hospital to the community and collaborate with GP practices.

Conduct assessments, develop care plans, and deliver interventions based on patient needs.

Collaborate with healthcare professionals, Health Care Assistants (HCAs), and other team members.

Attend regular in-service training, study days, and participate in service development activities.

Ensure accurate documentation and adhere to relevant policies and procedures.

Maintain a high standard of professional practice and provide compassionate care to patients.

Requirements:

Registered Nurse (RN) with a valid practicing certificate in New Zealand.

Previous experience in community nursing, primary health, medical nursing, or a related field.

Strong knowledge and skills in areas such as palliative care, wound care, and medical interventions.

Full driving license is required due to the nature of the position.

Excellent communication and interpersonal skills.

Ability to work autonomously and as part of a multidisciplinary team.

Flexibility to work morning shifts (8 am to 16:30 pm) and occasional weekends.

The district nurse team plays a vital role in supporting patients' transition from hospital to the community. They provide specialized care and interventions to patients in their homes, working closely with GP practices and other healthcare professionals.
