About the Employer : This organisation is a registered charitable trust who provide primary health care and community health services within the wider Wellington Region. Your responsibility: • To be a part of the Pacific Community Navigation service team, which will have staff located in Wellington and Porirua • To provide early intervention and facilitation to primary healthcare services and support lifestyle and personal change • To be a key contact person between Pacific fanau, general practice, primary health care teams, the Pacific community, and other stakeholders • Is to work with primary and secondary health care providers to identify the priority people who are not accessing primary health care at levels appropriate for their health need and support these fanau to get health care • Participate in Multi-disciplinary team meetings to provide a social work perspective to support individual/fanau care plans About you: • Resilience and capability to manage many demands in a professional manner • Able to work independently and show initiative and accountcapability for own actions • Demonstrates excellent interpersonal and communication expertise and can work with a wide variety of people in a variety of settings. • Can share how developing and implementing culturally responsive approaches for an organisation improves their capability to work with Pacific patients. • practice in working with Pacific fanau, knowledge Pacific views to influence better health for Pacific patients, fanau, and community.
