As a Support Facilitator, your role will be vital in conducting comprehensive needs assessments, identifying support requirements, and coordinating personalized support packages for our valued clients. By adopting a collaborative approach, you will work closely with clients, their families, and relevant stakeholders to ensure a thorough understanding of individual goals and needs, making them achievable. Key Responsibilities: » Conduct thorough needs assessments and prioritize client support requirements. » Coordinate and customize personalized support packages in collaboration with relevant parties. » Foster strong relationships with clients, their families, and internal teams. » Maintain accurate documentation and record relevant information in alignment with established practices and processes. » Enjoy the flexibility of working arrangements. About You: » Experience in the health, allied health, education, or disability sectors. » Strong verbal and written communication skills, with the ability to connect with individuals from diverse cultural backgrounds. » Ideally, previous experience in coordinating natural and formal supports and services for individuals with diverse needs. » Proficiency in navigating operational IT systems with a commercially savvy mindset. » Passionate about improving the lives of individuals with disabilities and committed to upholding our clients' values.
