Service CoordinatorJob location:Auckland All Auckland Work Type: Contract » » » • Fixed term function- 2 months- 2 locations in Auckland • Must be a RN/OT/Physio • Mon-Friday- 8:30AM- 5PM-income- $32-$34 per hour plus laptop & claim mileage The Company With 650 clients nationally and over 1000 employees/contractors this is one of the largest organisations in New Zealand providing support workers in the community. You’ll have the support of the DHB and complementary agencies (eg General Practitioners, Geriatricians, AT&R units, Community health team, physios, OTs) as well as a good network within the group from Auckland. The function Undertake clinical work with service users (using significant templates/documentation): • complete clinical assessments & re-assessments as required with non-complex clients delegated by Lead Service Coordinator. • draw up/review individual support programmes/ plans • deliver clinical support to non-complex clients • contact with other SC’s in the team who are registered health professionals. • Assist with conducting support worker meetings. • Ensure that service users are case managed against all significant clinical service user’s guidelines and best practices • Work under supervision of the Lead Service Coordinator. • Report changes in client needs to the Lead Service Coordinator. • Participate in peer review meetings. • Be competent in applying a systematic approach to executeing a comprehensive service user assessment. The Ideal Person We require the following: • You must be a registered health professional – a nurse, physio, OT with a current practicing certificate • Be physically fit and able to get up and down off the floor • Be self-motivated with the capability to juggle the ever-demanding workload This is a really compensateing position that requires lots of hard work, multi-tasking and problem solving.
