District Nurse - Community HealthLocation WellingtonNew Zealand Job type Permanent Salary 89,868 to 95,340 Category Medical > Nursing - Community, Maternal & Child Health » Full and part time positions available » No more night shifts! » $89,868 to $95,340 per annum We are currently sourcing for experienced Registered Nurses (RNs) for District Nurse positions in Community Health. Both Hutt Valley and Wellington locations. We have part time and full time postions available. Key Responsibilities: » Conduct comprehensive assessments of patients' health needs and develop individualized care plans. » Administer medications, treatments, and wound care according to established protocols and best practices. » Monitor patients' progress and adjust care plans as necessary to ensure optimal outcomes. » Provide education and support to patients and their families to promote health maintenance and disease prevention. » Collaborate with physicians, therapists, social workers, and other healthcare professionals to coordinate holistic patient care. » Maintain accurate and up-to-date patient records and documentation. » Participate in regular team meetings and contribute to quality improvement initiatives. » Adhere to ethical and legal nursing standards and guidelines. » Demonstrate empathy, compassion, and professionalism in all patient interactions. Qualifications: » Registered Nurse (RN) APC » Previous experience in community or home-based nursing preferred. » Strong clinical assessment and critical thinking skills. » Excellent communication and interpersonal abilities. » Previous experience in community nursing, primary health, medical nursing, or a related field. » Strong knowledge and skills in areas such as palliative care, wound care, and medical interventions. » Full driving license is required due to the nature of the position. » Ability to work autonomously and make sound decisions. » Proficiency in maintaining accurate and organized patient records. » Dedication to providing patient-centered care.
