Ko wai m?tou? – Who are we?
The name, Te Awakairangi, means ?esteemed' or ?precious' and was bestowed by one of the founding Trustees, Kara Puketapu, of Te R?nanganui o Te ?ti Awa in 2012. Te Awakairangi Health Network is a primary care network that provides quality primary care services across the Hutt Valley. We work with the general practices and a wide variety of healthcare providers and community organisations to empower people and enable them to receive the care they need.
Te Awakairangi Health Network is committed to meeting its obligations under Te Tiriti o Waitangi, working towards achieving equitable health results for M?ori in the Hutt Valley. Ensuring this delivery is enabled via the bespoke services we provide, develop, and implement in conformity with the Articles of Te Tiriti and the principles – Tino Rangatiratanga, Equity, Active Protection, Partnership and Options.
Te ?hua o Te T?runga – The nature of the position
The overarching objective of this newly created position is to make a positive difference to the health and wellbeing of wh?nau across the Hutt Valley. Guiding the organisation's work to improve M?ori health results, the function will navigate and lead mahi in population health. You will set about identifying opportunities within the Pae Ora environment to support locality planning and the development of provider networks, by facilitating partnerships, and promoting system wide change advocating for health promoting environments, pro-equity approaches and wh?nau-centric design and delivery.
You will support Te Awakairangi Health Network as we continue our commitment to Te Tiriti o Waitangi and to achieving equitable results. Enhancing relationships with Mana Whenua and M?t? Waka, Atiawa Toa Hauora Partnership Board and with Te Whatu Ora, Te Aka Whai Ora, and Manat? Hauora, you will both guide and navigate. Working closely with your Executive leadership abilities team colleagues, you will support collective efforts in implementing the planning, reporting, and monitoring of our work to improve M?ori health results.
Furthermore, you will be enhancing staff knowledge of Te Reo me ?na Tikanga and cultural competency and enhancing our communications.
You will lead the Population Health team as they develop programmes and avoidance initiatives that contribute to achieving equitable health results for M?ori, Pasifika, and priority populations in the Hutt Valley.
Seeking to increase the reach of these population health programmes, strengthening relationships and building avoidance capcapability across general practices, will see you working across health providers, social service providers, wh?nau ora providers and community organisations across the Hutt Valley.
Utilising your population health knowledge, will see you strengthen engagement with communities, providers, programme development and service delivery across the Network. Local knowledge and networks in the Hutt Valley with Iwi, M?ori and Pasifika communities would be very valuable.
Ng? tino P?kenga – Essential expertise/practices for the function
A tertiary qualification in public health, health or social service, or broad population health practice and knowledge is required.
Requisite practice, knowledge and competencies necessary in this position are to include;
   » An knowledge of the New Zealand health sector, current primary health care and population health issues, and the barriers to equity and good health results.
   » An knowledge of how to use a Treaty of Waitangi focus and an equity lens when designing and delivering programmes and services.
   » Proven relationship management expertise, with the capability to lead and facilitate change.
   » People leadership abilities practice.
   » Knowledge of Te Reo me ?na Tikanga.
   » practice in programme design that improves health results for M?ori and priority populations.
   » A Full current driver's licence is required.
Past practice working within priority populations such as; M?ori, Pacific peoples, people with disabilities, low salary, refugees and LGBTQIA communities will be required to be well demonstrated within your working history.
Renumeration for this function is positioned to attract individuals with a high level of cultural knowledge, a demonstrated strategic skill set, proven stakeholder relationships and practice working within the Health Sector in New Zealand.
