The function: Professional Responsibility: » Practices in conformity with legal, ethical, culturally safe and professional standards. » Maintains and develops own clinical skills and expertise in specialty nursing practice. » Fosters inquiry and critical thinking amongst colleagues to advance nursing practice and patient/client care. » Participates in peer review/feedback. » Pro-actively participates in own performance development and review. » Attends educational opportunities significant to staff nurse function and scope of practice. Management of Nursing Care » Supports patients, family and wh?nau to manage medical therapy prior to surgery on direction of the multidisciplinary team. » Provides co-ordination of surgical admissions – including referral management, and pre-screening assessments. » contacts with patients, family and wh?nau and the PCCS multidisciplinary team to ensure that patients’ pre-surgery assessment occurs relative to scheduled surgery. » Provides a central reference point for cardiac surgery scheduling and disseminates timely information delivery to key stakeholders. » Participates in PCCS cardiac surgical conferences. » Works collaboratively with the PCCS multidisciplinary team and ADHB overseas coordinator in planning for international patients. » Uses nursing expertise and skills to assess, plan, implement and evaluate patients/ clients/ family / wh?nau health needs. » Provides direct nursing care for patient/clients to achieve best health results. » Evaluates clinical results and reports variance to clinical pathway, protocols or best practice guidelines. » Educates and provides information to patients/clients/family/ wh?nau to improve expertise of disease/illness, self-management and avoidance of complications and promotion of recovery. Facilitates education for patients, family and wh?nau pre-operatively in close collaboration with the Nurse Specialist team. » Contributes to effective discharge planning and/or referral processes to health care providers and support agencies to meet identified health needs. » Accurately documents assessments of patients/clients health status, diagnosis and response to nursing and others interventions and treatments. » Applies diagnostic reasoning and professional judgement to nursing practice issues/ decisions. » function models culturally safe nursing practice. » Fosters the provision of positive patient/client results and person-centred care.
