Are you passionate about working with people? Have you had experience in working closely with the Maori and Pasifika communities?

We have a great opportunity available for people within Kirkirroa to work closely with the Maori and Pasifika communities to work with and assist whanau with relocating from temporary to permanent housing, along with providing housing and wellbeing service coordination to meet whanaunga needs. While you will be supporting new and existing whanaunga, you'll also be responsible for developing relationships with government agencies and care providers to ensure the whanaunga are receiving the help and support they need to get ahead in life.

As a housing navigator, you'll be responsible for :
   » Reviewing whanaunga documentation prior to establishing contact
   » Explaining your role to whanaunga, enforcing the need for constant engagement to remain in temporary housing
   » Engaging in practical tasks to build rapport with new and existing whanaunga
   » Plan initial clinical and needs assessment contacts to be conducted in an individualized way that suits the whanaunga way of engaging
   » Analysis of intake assessment and risk assessment material to avoid duplication and cross
over
   » Engage with other health/well-being providers who may be engaged with whanaunga to
facilitate coordinated, collaborative plan
   » Developing a plan with the whanaunga that covers practical immediate help, support during transitions, future tenancy set up, and sustainability
   » Work alongside Kaitautoko - Housing pathways to assist whanaunga to navigate the MSD Housing Assessment process ensuring the person's situation is adequately prioritized on the waitlist
   » Enable access to other support and wellbeing agencies to assist
   » Maintain close communication with Kaitautoko within the team, exploring issues that they may have observed as issues but are outside their role
   » Work with the whanaunga, if required, for up to 9 months to ensure tenancy sustainability and other required support services remain engaged
   » Engage in quality improvement reviews of practice with the team as a result of outcomes of service delivery monitoring
   » Support during the transition and beyond to deal with other issues if practical or service coordination to ensure whanaunga has access to services to assist in areas of need

To be successful in your application, you will need to have:
   » An ability to build rapport with a wide range of personalities
   » Previous work experience with people require mental health support
   » Have good time management skills, with the ability to plan out your day ahead
   » A great attitude with the ability to work with a unique, wh?nau first, Tikanga based, wellbeing approach organization.
   » The ability to analyze whanaunga and provide reports as required
   » Have the ability to build rapport with internal and external support staff
   » A full Class 1 driver's license with no driving offenses
   » Two satisfactory work references
