They provide equitable, compassionate, free-of-charge hospice care working primarily with patients, families and carers in the place they call home, and at their Inpatient Unit. As an organisation they are committed to doing the work necessary to improve results for M?ori and Pacific people in their care. About the function This Wellington Social Worker function provides continuing support to patients, families and the multidisciplinary team by working closely with the patients and their wh?nau who are utilising these services within the Wellington community, and by contributing to a multi-disciplinary approach to care. This key function provides professional and skilled social work services and contributes towards the wider knowledge and knowledge of hospice care in the Wellington community. About you - practice working in a community setting preferable. - Appropriate tertiary qualification in Social Work. - Qualified and registered Social Worker. - Demonstrated up-to-date knowledge of socio-political situations and their impact on patient, family and wh?nau groups. - Full current driver’s licence.
