The Company
Our client looks after people in the community who need support and love the most. Their service delivers a high-standard of care to the elderly and those with a chronic illness or discapability, so they can continue living at home as long as possible. The function:
This is not a desk job; in this function, you will be out in the community each day visiting a few clients. At each client visit, you meet with the client and sometimes their family, go over any doctors notes, and then create and supervise an individual care plan for each client. Your job is to design these care plans, and then direct and delegate the Support Workers who will look after the clients as instructed.
 
About you:
   » Must be a Registered Health Professional – such as a Nurse, Occupational Therapist, or Physiotherapist, or a qualified social worker.
   » practice in a home healthcare or aged care setting is ideal
   » Time management expertise.
   » Excellent at planning, prioritising, and juggling
   » Kind and calm presence, empathetic, and strong interpersonal expertise.
   » Excellent analytical and problem solving expertise
   » Patient, responsive, and flexible.
   » Full Class 1 Licence
   » Dedicated to continuing education and professional development.
 
The income range for this position is $71k - $84k located on practice.
