About the company This organisation is a local charity that provides specialist palliative care services for people living in Wellington, Porirua and K?piti. They provide equitable, compassionate, free-of-charge hospice care working primarily with patients, families and carers in the place they call home, and at their Inpatient Unit. As an organisation they are committed to doing the work necessary to improve results for M?ori and Pacific people in their care. About the function These functions are primarily providing care within the community and in-patient setting but may, at times, be required to provide back up support to Palliative Care Coordinators providing care to patients in the community. The Registered Nurse plans, co-ordinates and implements palliative nursing care for the patient, wh?nau and carer. They also work as a key member of the multidisciplinary team to ensure that the continuum of care for the person and family can be delivered across multiple sites using collaborative planning and implementation with a range of health care providers. About you - Hold a current practicing certificate. - A minimum of bachelor's degree in nursing with two years full time equivalent post registration clinical practice preferably in palliative care nursing. - Have a strong affinity with multidisciplinary teamwork, influencing patient/family/wh?nau focused practice and goal attainment.
