About the Employer : This organisation is a registered charitable trust who provide primary health care and community health services within the wider Wellington Region. Your responsibility: • You will work with the Pacific Navigation service team which has staff located between Wellington and Porirua • You will be a key contact person to help contact with general practice, primary health care teams, the wider Pacific community and other stakeholders • To work with both primary and secondary health care providers to identify those not accessing health care and prioritising support located on their needs • To participate with multi-disciplinary team meetings and supporting care plans • To provide early intervention and facilitate engagement with health care services • To facilitate activities to help support lifestyle change • Supporting individuals and the wider community in knowledge health needs and how to access appropriate services About you: • Good clinical expertise, practice and clinical assessment. • Demonstrates excellent interpersonal and communication expertise and can work with a wide variety of people in a variety of settings. • Can share how developing and implementing culturally responsive approaches for an organisation improves their capability to work with Pacific patients. • practice in working with Pacific fanau, knowledge Pacific views to influence better health for Pacific patients, fanau, and community.
