The programme enhances the care provided by the main carers and other health care professionals in the last and precious days of life. This fulfilling and important function is » located in patients’ homes » To provide autonomous expert clinical palliative care support to ensure all our end of life community care patients receive a high standard of care » To support carers with overnight respite to enable them to continue caring for their loved one during the day at this most valuable time.
