The function » The Service Co-Ordinator supports their Lead in the delivery of a user driven, flexible, integrated community-located support service for older people who have been assessed as requiring home support. This restorative home support service works with service users to maintain or build independence. » A Temp opportunity for 3-6 months, but if interested later in a Permanent fulltime function too, this is also possible and involves further involvement eg with Support Workers etc » Note You will use your own car receiving mileage; but later, given permanent opportunity, most elect for a company car Main tasks Service Coordination » Support the multidisciplinary service provider team in the delivery of a service user-driven, flexible, integrated, community-located support service. » Deliver the service model, meeting agreed upon service goals and assist Area Manager with change management processes » Develop a culture that encompasses the values and vision of the organisation and the service, and ensure goals have been set/support plans are current » You will schedule service provision, informing all parties, contact with clients re home and community support and any updates/changes » Enter required documentation Clinical Competency » Undertake clinical work with service users (using significant templates/documentation), complete clinical assessments & re assessments, deliver clinical support, draw up/review individual support programmes/ plans as required with non-complex clients » contact with other SC’s in the team who are registered health professionals. » Assist with conducting support worker meetings. » Ensure that service users are case managed against all significant clinical service user’s guidelines and best practices, working under the Leads supervision, reporting any changes in client needs Further tasks » Relationship Management » Reporting & Systems » Health & Safety You » You will be a Registered health professional (Nurse, Physio, OT) with a current Practicing Certificate » Have demonstrable negotiation and conflict resolution expertise » Clinical assessment and documentation expertise » Well developed oral and written (report) presentation expertise with competency in Microsoft Office i.e. Word, PowerPoint, Excel, Outlook….for the needs assessment function, the InterRAI contact tool is used when assessing in the community » Full Drivers Licence, own car advantageous Hours » A Temp function via our Agency, you will be working flexible fulltime hours Monday to Friday - on the field yourself the majority of the time, working/going out in the house, in their central/south office etc. Three client visits a day can be typical.
