We have an opportunity for an individual with excellent interpersonal and relationship management expertise to join the Whakatane MSD team as an Integrated Services Case Manager (ISCM) on a permanent basis. this function works alongside our individuals and whanau who have presented with complex needs and require an intensive wraparound service. The focus of this position is to help identify the needs of the whanau, covering these immediate needs and then linking them with appropriate services and agencies. The ISCM position will see you working closely with key agencies and community groups to build comprehensive plans that put our people in the centre. Remuneration for this function starts at $71,231 per annum (Band 5) dependent on significant work practice, evidenced expertise and other factors.
