Social Worker - Hours negotiableLocation WellingtonNew Zealand Job type Permanent Salary to Category Medical > Other » Permanent role » Community and Inpatient » Hutt Valley location Adecco are excited to be supporting our client to recruit a Registered Social Worker for the Older Persons Mental Health Service - Community and Inpatient The opportunity: Caring for our older people you will be working alongside a multidisciplinary team as a registered social worker in the community team, you will be supporting patients who are temporarily admitted to the older patients ward. Some of the roles will being undertaking social work assessments, supporting the client's pathway, discharge planning and the coordination of resources. You will also be a key point of contact for wh?nau. There is an amazing passionate team ready to welcome you onboard! The Older Persons Mental Health service is for the most part based in the community visiting clients in their homes or aged care facilities Duties include: » Giving OPMH patients access to efficient, high-caliber social work services » Collaborating with clients and whanau to create interdisciplinary treatment objectives » Completing thorough evaluations and plans jointly with customers and whanau » Providing social work intervention, both directly and indirectly, to assist clients in their treatment and recovery the multidisciplinary team procedure, cultural diversity is given fair evaluation. » Assisting clients and whanau to achieve optimal health, well-being and safety through provision of appropriate assessment, treatments, rehabilitation and education » Implement suitable assessments, treatments, rehabilitation, and education, to aid clients and their families to attain maximum health, well-being, and safety. » Collaborate and network with community organizations to accomplish the goals established through the multidisciplinary team process. Requirements: » NZ Registered Social Worker with a current APC » The capability and assuredness to work independently with complex cases. » Ideally a minimum of 5 years' experience in a similar clinical role » Outstanding communication and networking skills » Able to work in a transdisciplinary setting along with other health and community based professionals » Knowledge of cognitive impairment, issues of capacity and legal frameworks » Strong commitment to excellent clinical and professional practice » Commitment and understanding of the Treaty of Waitangi » Current full NZ drivers licence » A good command of the English language both written and spoken APPLY NOW or to find out more call Angela on 021861454 Adecco is acting as an Employment Business in relation to this vacancy.
