MDHB has a vision of transferring specialist care from hospital to the community, upskilling primary care in the process and fostering a collaborative approach to managing older people with frailty. To this end a recent review of the medical staffing on the Horowhenua Health Centre ward (STAR 4) has resulted in a new medical model of care. STAR 4 is a 20 bedded ward, with a mix of rural and Assessment, Treatment and Rehabilitation (ATR) beds for patients of all ages, although patients are predominantly over 65 years old. The ward also has special skills in palliative care. Our medical cover roster is for four 0.70 full time equivalents (28 hours per week) to provide inpatient, outpatient and community care. The function will also cover leave and contribute to the 1 in 4 on call roster. There is support from a geriatrician who is on-site three days a week. As a Horowhenua Health Centre ElderHealth Medical agent you will also be part of the wider ElderHealth medical team and service. This team consists of eight geriatrician senior medical agents and one medical agent who are located at Palmerston North Hospital, in addition to the medical agents at Horowhenua Health Centre. A multidisciplinary approach to the management of care is ensured with the support of senior nursing and therapy staff. As an employee of MidCentral District Health Board you will be eligible for generous annual leave, continuing medical education and related leave, superannuation scheme, etc. You may also have the opportunity to contribute to the General Practice Services within the Horowhenua region. Levin is the largest town and seat of the Horowhenua District, in the Manawatu-Wanganui region of New Zealand's North Island. Our location in the centre of the lower North Island puts us in close proximity to excellent hiking and mountain bike trails, fishing rivers and we are only a short drive from the beach. Wellington (with the buzz of our capital city), the Wairarapa and Hawke's Bay (wine growing regions), and Mt Ruapehu (ski fields) are only a couple of hours away.
