The function You will manage a case load of between 15-30 Support Workers and 160-180 clients across the Tauranga region. Your clients will include the aging population and people with debilitating injuries. There is full flexibility to manage your day as you see fit. Your workload will include: • 1-2 client visits per day – these could be new clients or changes to the needs of existing clients • Coordinating with the DHB team with new referrals or complementary agencies to support your clients with their needs • Creating individual support plans, administration and delivery to the highest standard • Staff management – including recruitment, inductions, training, monitoring and managing disciplinary situations with your team this function is contract, while the position is recruited permanently. Main tasks: • Service Coordination Support the multidisciplinary service provider team in the delivery of a service user-driven, flexible, integrated, community- located support service. • Deliver the service model, meeting agreed upon service goals and assist Area Manager with change management processes • Develop a culture that encompasses the values and vision of the organisation and the service, and ensure goals have been set/support plans are current • You will schedule service provision, informing all parties, contact with clients re home and community support and updates/changes • Enter required documentation • Clinical Competency Undertake clinical work with service users (using significant templates/documentation), complete clinical assessments & re assessments, deliver clinical support, draw up/review individual support programmes/ plans as required with non-complex clients contact with other SC’s in the team who are registered health professionals. • Assist with conducting support worker meetings. • Ensure that service users are case managed against all significant clinical service user’s guidelines and best practices, working under the Leads supervision, reporting any changes in client needs • Further tasks Relationship Management Reporting & Systems Health & Safety You: • You must be a registered health professional – a registered or enrolled nurse, physio or OT.
