The programme will provide individualised community-located services aimed at supporting people living with dementia to remain independent in their homes for longer while supporting and empowering them to live their best lives. This exciting new function will see you utilise your expertise and practice as you support our clients living with dementia through the journey, you will become their dementia champion.
