The function:
Are you a registered nurse looking for an exciting opportunity? Are you looking to work for an organisation which values continuing development of its people? Our client is introducing a new innovative dementia programme into New Zealand. The programme will provide individualised community-located services aimed at supporting people living with dementia to remain independent in their homes for longer while supporting and empowering them to live their best lives. This exciting new function will see you utilise your expertise and practice as you support our clients living with dementia through the journey, you will become their dementia champion. This involves:
• Designing and implementing a work programme for our dementia care services.
• Working collaboratively with our dementia clients and families
• Development of individual care plans, in conformity with the clients’ specific needs
• Initial assessment and regular reassessment of our dementia clients
• superviseing delivery of Care Plans by Support Workers
• Building key relationships with dementia providers to ensure your clients living with dementia have access to the optimal health & social support required.

You:
• Have a current Practising Certificate
• practice in aged care and in particular dementia sector.
• Compassion and passion for people living with dementia; in particular improved care for those living with dementia
• knowledge around types of dementia, presentations, stages and appropriate care plans.
• practice liaising with medical, other health professionals and networks of carers in the dementia sector.
• Advanced assessment expertise and capability to create care plans
• Medication Management
• Capcapability to prioritise and manage your time effectively, with well-developed organisational expertise
• You embrace the flexibility and challenge of being a cornerstone member of a new programme
