Covid Community Clinical function - Registered NurseJob location:Wellington Wellington Central Work Type: Temporary » » » »Company car to go on community home visits »Whanau-oriented approach »Supportive team About the company The client is a primary care network and not for profit social enterprise which provides quality primary health care services to an enrolled population of 331,000 people across the Wellington, Porirua, Wairarapa and Kapiti areas. About the function The key purpose of the function is to provide day to day support to the community and whanau who have come into contact with Covid-19, as well as assessment, care and preventative measures for the community and whanau during the pandemic. You will work in partnership with clients, family/whanau, general practice teams and other health providers, to ensure that the clients access the support and services they require. You will offer efficient and effective support to the community and people who are isolating at home and engage with those who are in home isolation or providing support to those in home isolation, including home and community clinical assessments, testing, vaccinations and other supports where required. this function may also need to provide cover as needed across other Covid areas. You will be utilising a Whanau Ora approach when undertaking health assessment and care planning processes in partnership with client/family/whanau and significant health care providers, providing individual and group self-management, and self-care support and working in the community, and from the office (Phone calls, home visits and community-located clinics and events). The shift are rotating, rostered between the hours of 8am to 8pm, 7 days a week, to cover service requirements. About you You must have: • Clinical background with a current practicing certificate or similar • Minimum of 3-years post-graduation practice preferred • Current full driver’s licence.
